On behalf of CHS Coaching staff, please read and complete the followings otherwise you will not be allowed to participate in tryouts!

Before you can participate in try-out you must have the following handed in to the CHS Athletic Office:

• Physical Confirmation Form
• Signed Athletic Code

- Athletic Code Signature Form Link (http://www2.caledonia.k12.mi.us/athletics/athcodesignaturepg11-12.pdf)
• $90 Participation Fee

It may be a good idea to contact the athletic office (616.891.0211) to confirm someone will be there prior to making the drive.
